- ACL reconstruction and meniscal surgery
- Rotator cuff and shoulder arthroscopic surgery

- Relevant examination findings
- Current X-rays, ideally taken within the last six months
- Previous treatment and response
- Medication list and relevant medical history
- ACC number and date of injury where applicable
